How do you solve the Patient Initials Wheel in Townfall?
Route only O−, O+, A− and A+ to the centre, then spin the rings so straight lines connect the initials T.F., H.M. and S.W., and confirm with the ORDELL button. Your blood type — needed to know which initials to keep — comes from the Medical Wristband in your Equipment plus the surname on the display.
What happens after the treatment sequence in Townfall?
The Otherworld shift: the hospital becomes 'Medical Centre (Different)', a separate layout with its own map, ward-letter navigation and the two-stage blood-routing puzzle that ends with the transfusion — the Many Into One trophy.

What happens in Part 3?

The Patient (chapters 7–8) is Townfall’s medical centrepiece — the most interior, most linear Part so far, which is good news for collecting: there’s very little to walk past. Simon reports to Dr. Glenn for A Long Night Ahead, and the Part then plays the Medical Centre in two states: the normal building first, the (Different) Otherworld version after the shift — each with its own map.

What are the examination settings?

The treatment sequence — Zoe’s patient section, continuing her medical role from Part 2 — is a chain of small instrument puzzles. Match the machine in front of you; several look similar:

What are the Medical Centre’s two puzzles?

The Patient Initials Wheel — blood groups and rotating rings. Route only O−, O+, A− and A+ to the centre, which leaves T.F., H.M. and S.W. in play; connect exactly those three with straight lines and confirm with the ORDELL button. The answer is personalised: the blood type that says which initials to keep comes from the Medical Wristband in your Equipment plus the surname on the display. If the wheel won’t confirm, you’re missing the wristband — it is an Equipment pickup earlier in the building and cannot be recovered after the shift. It is not brute-forceable.

The locked hallway door — interacting with the locked door at the end of a hallway yields the Room 204 Key, which carries you deeper into the building and toward the transition.

Also grab the Plank from a side room during Chapter 7 — a melee weapon in a section otherwise built around stealth.

How does the Otherworld shift change the hospital?

After the treatment arc the building becomes Medical Centre (Different) — same shell, rearranged wards, and its own map on the counter near the upper-floor metal door. Navigate by ward letter + floor + room: Ward W’s lower room 13 ladders up into Ward S, and Matthew’s bag drops between floors into Ward P. Two things change how you play here: your kill count starts mattering — kills now feed the ending ratio — and the Otherworld rooms hold documents that only exist in this layout, so sweep again rather than assuming the building is cleared.

The gate at the end is the two-stage blood puzzle — patient list, ward keys, blood bags, donor dial, ending in the transfusion and Many Into One. Full sequence: blood puzzle.

What are the traps in Part 3?

  • Save It For The Patients — no healing through Zoe’s Rounds and the Patient sections. Carried in from Part 2; fails silently.
  • Two sealed areas — the normal hospital closes at the shift, the Otherworld at the transfusion. Sweep documents in both.

After the transfusion the story moves to the union estate — the Part 4 walkthrough picks up in Clara Woods.
