Two design choices keep the platinum cheaper than the 37-trophy list implies: cumulative counters carry between playthroughs, and manual saves at the end of Part 4 and Chapter 15 collapse four endings into reloads.

How tight is the speedrun trophy?

Fleeting Visit asks for a full clear in under 4 hours — and it’s very lenient: since a story-only run is under 2, you can fold the speedrun into a run you were doing anyway. The catch is that the timer keeps running through cutscenes and every menu except the pause screen, so pause if you need to check a guide, and turn on the display via Game Options → Show Time Played. If it still feels tight, the Pistol-3000’s one-shot damage makes a dedicated later run trivial.

What stretches the time most?

Three things: the no-backtracking structure (a missed collectible is a run-slowing second visit to that area next run), the two NG+ joke endings that force the second playthrough, and your own document discipline — the Clues & Documents count is invisible mid-run, so keeping a tally saves more time than any shortcut.
