Where is the blood puzzle in Townfall?
Inside Medical Centre (Different) — the Otherworld version of the hospital that Part 3 shifts into once the treatment sequence ends. Its map is a separate pickup on a counter near the upper-floor metal door.
How do wards work in the Otherworld Medical Centre?
Navigation is ward letter + floor + room number. Ward W's lower room 13 contains a ladder up to an upper room in Ward S, and Matthew's bag drops between floors into Ward P — so read room labels in order rather than following corridor instinct.

What is the blood puzzle?

The blood-routing sequence is Part 3’s final gate — the puzzle the Medical Centre (Different) builds to once the Otherworld shift rearranges the hospital. It is a two-stage lock: the game wants the wards physically opened and the blood physically routed before the donor dial matters. Everything about it plays by the Otherworld’s own layout, which is why its separate map is worth grabbing first. (The normal building’s own gates — the Patient Initials Wheel and the Room 204 door — sit before the shift and are covered in the Part 3 walkthrough.)

How do the wards connect?

The (Different) hospital navigates by ward letter, floor and room number — not by the corridors you remember from the normal building:

  • Ward W, lower room 13 — contains a ladder giving access to an upper room in Ward S.
  • Matthew’s bag — drops between floors and lands in Ward P.

Reading labels in order is the whole trick: a room number without its ward letter sends you to the wrong corridor every time.

How do you finish the sequence?

With the patient list in hand and every ward open, the second stage is mechanical: place the blood bags at the marked stations, then set the donor dial to complete the routing. The sequence closes with the transfusion — accepting it earns the Many Into One trophy and pushes the story into Part 4’s Clara Woods estate.

Two cautions for this stretch. The Save It For The Patients trophy still applies — no healing items during the Rounds and Patient sections, and it fails silently.
